Responsibilities include: Evaluate residents to determine speech therapy needs, functional limitations, and rehabilitation potential under physician orders.

Develop, implement, and update individualized treatment plans.

Provide treatment for communication, cognitive-linguistic disorders, and swallowing disorders.

Collaborate with physicians, nursing staff, residents, families, and the interdisciplinary care team.

Participate in care conferences, utilization review meetings, and rehabilitation meetings.

Educate residents, caregivers, and families on treatment strategies, safety, swallowing precautions, and home programs.

Complete timely, accurate, and compliant documentation using electronic medical record systems.

Assist with restorative nursing education and staff in-service training.

Maintain a safe treatment environment following infection control and safety standards.

Participate in quality improvement initiatives and rehabilitation program development.

Qualifications

Master's Degree in Speech-Language Pathology from an accredited program. Eligible for California Speech-Language Pathology Clinical Fellow licensure. Eligible to complete Clinical Fellowship requirements for ASHA Certificate of Clinical Competence (CCC-SLP). Current CPR certification (or ability to obtain upon hire if required). Strong communication, organizational, and interpersonal skills. Experience with electronic medical records is preferred (Net Health and PointClickCare experience is a plus). Physical Requirements This position requires frequent standing, walking, reaching, lifting, pushing, pulling, and assisting residents with mobility and therapeutic activities. Candidates must be able to safely perform the essential duties of the position while maintaining a safe environment for residents and staff. Requirements

Candidates must hold a Master's degree in Speech-Language Pathology and be eligible for California licensure as a Clinical Fellow. Strong communication skills and the ability to perform physical tasks related to patient care are required.
