We are seeking a compassionate, dedicated, and professional Registered Nurse (RN) to join our Home Health and Hospice team. The RN will provide skilled nursing care to patients in their homes or hospice settings, ensuring high-quality clinical outcomes, patient safety, and comfort. The ideal candidate demonstrates clinical expertise, empathy, and strong communication skills while promoting patient-centered care and interdisciplinary collaboration.
Current and valid Registered Nurse (RN) license in the State of Texas (or compact state). Graduate of an accredited School of Nursing (BSN or ADN). Current CPR (BLS) certification required. Valid Driver’s License, auto insurance, and reliable transportation required.
Candidates must hold a current and valid Registered Nurse license in Texas and possess a current CPR (BLS) certification. A minimum of one year of clinical nursing experience is preferred, along with a valid driver's license and reliable transportation.
Minimum one (1) year of clinical nursing experience preferred (home health or hospice experience strongly preferred). Knowledge of OASIS and EMR charting preferred for home health nurses. Hospice RNs should have knowledge of symptom management, palliative care, and end-of-life protocols.
Patient visits in home, assisted living, and hospice settings. Requires driving to patient homes and facilities. Flexible schedule including possible weekends and on-call duties. Must be able to lift, bend, and assist patients as needed.
