Overview Geriatric Advance Practice Provider role in value-based care model.

Monday-Friday, no weekends or evenings schedule.

Salaried role with strong annual quality-based bonus.

Patient panel across Denton, Aubrey, Lewisville, Carrollton areas with 30-45 min patient visits seeing approx.

6-9 patients per day.

Responsibilities

The Nurse Practitioners we seek are those who have directly related experience in home health, geriatrics, hospice and primary care. The Nurse Practitioner who wants to practice medicine with a focus on patient care, not volume. We want our clinicians to take the time needed to truly address the patient’s needs.

Qualifications

Licensed as a NP in the State where HarmonyCares Medical Group office located Federal DEA License/eligibility Geriatric training/experience preferred Active CPR/BLS Certification Board Certification Must maintain a valid driver’s license and good driving record Posted Max Pay Rate Posted Min Pay Rate Pay Transparency Individual compensation packages are based on various factors unique to each candidate, including skill set, experience, qualifications, and other job-related considerations.

Requirements

Must be a licensed Nurse Practitioner with a valid DEA license and CPR/BLS certification. Experience in home health, geriatrics, or hospice care is required.
