Working alongside an experienced interdisciplinary team, you'll provide individualized physical therapy services that help adult and geriatric patients regain mobility, improve function, and maximize their independence in a skilled nursing and post-acute rehabilitation setting.

What You’ll Do Evaluate patients and develop individualized plans of care Deliver evidence-based physical therapy focused on improving mobility, balance, strength, safety, pain management, and functional independence Collaborate with physicians, nursing staff, families, and interdisciplinary team members to achieve the best patient outcomes Supervise Physical Therapy Assistants in accordance with state licensure requirements Complete timely, accurate documentation supporting quality patient care and regulatory compliance
Graduate of an accredited Physical Therapy program Current Physical Therapist license (or eligibility for licensure) in Minnesota
