Position purpose

The Licensed Practical Nurse (LPN) is responsible for providing comprehensive nursing oversight, coordination, and direct care to individuals served within the program. This role supports all health-related needs through assessment, intervention, monitoring, education, and collaboration with interdisciplinary team members, family members, and healthcare providers. The LPN plays a vital role in promoting the physical and emotional well-being of participants while ensuring compliance with healthcare regulations and agency standards.

Major responsibilities

Maintain accurate nursing documentation, medication records, and electronic health records.

Conduct face-to-face and telephone interactions with program participants as needed.

Complete nursing assessments within 24 hours of a participant's admission to the residence.

Coordinate refills of non-controlled medications in consultation with the program psychiatrist.

Facilitate and support residential telepsychiatry appointments through virtual platforms.

Educate participants regarding their medications and treatment plans.

Ensure medications are administered as prescribed and documented in the electronic Medication Administration Record (eMAR).

Monitor participants for medication effectiveness, side effects, and adverse reactions.

Communicate effectively with nursing staff, program staff, physicians, and other healthcare providers.

Report significant health concerns and incidents to the Nurse Manager and Program Director.

Conduct and complete medical mobile services and mobile crisis interventions in accordance with applicable regulations and program requirements.

Requirements

Candidates must hold a current Pennsylvania Licensed Practical Nurse (LPN) license and possess valid CPR and First Aid certifications. A valid driver's license and successful completion of state and federal background clearances are also required.

Qualifications

  • Required
  • Current Pennsylvania Licensed Practical Nurse (LPN) license in good standing.
  • Current CPR and First Aid certification.
  • Valid driver's license with an acceptable driving record.
  • Pennsylvania Act 33 and Act 34 clearances.
  • FBI clearance, when applicable.
  • Completion of all training requirements associated with licensure and agency standards.
  • Successful completion of a physical examination, TB test, and Hepatitis B titer.

  • Experience working with individuals receiving behavioral health or community-based services.
  • Strong communication, organizational, and documentation skills.
  • Ability to work independently and collaboratively within a multidisciplinary team.
