Education & Experience Education and Experience Required: Master's Degree or higher in Social Work (MSW) and Licensed Social Worker in the State of Mississippi.

Responsibilities
Conducts and documents detailed psychosocial assessments/evaluations to interpret social, environmental, and medical needs of the patient/family. Meets the needs of transplant applicants and donors and/or level ii evaluations to determine mental illness/mental retardation discharge needs. Documents according to departmental guidelines and standards, prepares reports and maintains records as needed. Reassesses, monitors, and documents needs and interventions. Ensures that medical coverage is obtained by securing the appropriate documentation and materials. Evaluates the effectiveness of the plan of care and recommends alternative plans as needed. Provides or supervises plan of care interventions to include resource and referral assistance, crisis intervention, prevention, education, patient advocacy, bereavement, therapy, and counseling as appropriate. Collaborates with the healthcare team and involves the patient and/or family in the development and implementation of plans. Collaborates with other healthcare team members to facilitate the interdisciplinary care of patients and achievement of positive outcomes. Provides or supervises plan of care interventions to include resource and referral assistance. Recommends referrals and allocates community resources as appropriate.
