From our humble beginnings as a gathering of parents and concerned citizens who wanted to provide educational and training opportunities for persons with developmental and intellectual disabilities, The Resource Center has grown into a comprehensive agency providing services to thousands of persons with all types and levels of disabilities.

The Resource Center is the largest non-governmental employer in Chautauqua County with more than 1,300 employees on its payroll.

The mission of The Resource Center is to support individuals with disabilities and other challenges in achieving maximum independence, contributing to their community, experiencing lifelong growth, and enjoying quality of life.

LICENSED PRACTICAL NURSE (LPN) - JOB DESCRIPTION RESPONSIBLE TO: Registered Nurse Manager / Registered Nurse Supervisor FUNCTION: Provides on-site nursing services and health-related support to people at assigned sites.

The LPN is responsible for promoting the health, safety, and well-being of people supported through the delivery of nursing care, monitoring of medical conditions, and implementation of physician-ordered treatments and services.

Working under the direction of a Registered Nurse, and in collaboration with the interdisciplinary team, the LPN ensures that all health-related services are delivered in accordance with applicable regulations, professional standards of practice, and agency policies and procedures.

REQUIRED KNOWLEDGE, SKILLS AND ABILITIES: Knowledge of nursing principles and practices within the scope of a Licensed Practical Nursing, including medication administration, infection control, and basic health assessment.

Working knowledge of regulations and standards under the New York State Office for People with Developmental Disabilities, as well as applicable Department of Health and Medicaid requirements.

Understanding of common medical, physical, and behavioral health conditions affecting people with developmental disabilities.

Knowledge of person-centered planning and implementation of Individualized Plans of Protections, Plans of Nursing Services, and Care Plans.

Familiarity with incident reporting requirements, including reportable events and immediate protective actions.

Knowledge of documentation standards and electronic health records systems.

Basic understanding of interdisciplinary team roles and coordination of care within a certified site-based setting.

Requirements

Candidates must hold a valid New York State license as a Licensed Practical Nurse and possess current CPR and First Aid certifications. A valid driver's license and the physical ability to assist with lifting and transferring residents are also required.

OTHER DUTIES: Refer to Appendix REQUIRED TOOLS: Refer to Appendix 5/2007; 3/22, 07.28.26 IDD Services The Resource Center Union Grade: LPN FLSA Status: Hourly PTO/CAT Per CBA CFR: 316 Licensed Practical Nurse (LPN)–APPENDIX OTHER DUTIES CLINICAL: Assists the Registered Nurse in the training of staff, including the Agency Medication Administration Course. Ensures all immunizations are up to date and documented in the electronic health record. Contacts Providers as needed and ensures follow up is received and all contact is documented timely. Ensures lab work is completed as ordered. Ensures documentation occurs according to policies and procedures. Ensures diagnostic testing is completed as ordered. Reviews results and ensures all follow-up is completed timely. Ensures all referrals (OT, PT, Speech, Provider) are scheduled timely, and follows up on all referrals and adaptive equipment needs until a response is received. Document each encounter. Reads medical/health notes and offers feedback to staff regarding the entries. Communicates with Physicians regarding medical issues for people supported and documents conversation timely. Ensures any follow up is completed timely. Visits people in the hospital and communicates with the hospital medical team. . Documents visit timely, and reports update to the interdisciplinary team. As requested by Registered Nurse, provides support to produce medical documentation, including but not limited to: Surrogate Decision Making Committee, Psychiatric Consults, Medication Reviews, and any other designated medical documentation. Monitors documentation of Bowel Tracking, vitals and weights and other pertinent medical documentation, and follows up as necessary. Carries orders as directed/approved by the Registered Nurse Drop off/pick up new orders at Providers offices as needed. Obtains paperwork from Medical Records as needed for Emergency Department/Hospital visits Ensure all MOLST appointments are scheduled every 90 days Supports people on medical appointments when necessary, or as assigned. Utilizes least restrictive support techniques to assist people to check in at the appointment and to communicate their needs/concerns with the Provider. Ensures medication room is clean and organized and conducts Medication Administration Record reviews. Maintains prescriptions and follows up with pharmacy and/or physician for any expired or incorrect prescriptions. Orders medications from pharmacy as needed. Ensures site management is aware of upcoming appointments and reason for appointment. Ensures appointment consultation form and associated paperwork is completed prior appointment. Communicates with necessary Interdisciplinary Team members for Psych appointments. Ensures all consults, Provider orders, Labs, etc. that were received at the appointment or received via other communication modes (email, fax, etc) is uploaded to the electronic health record. Ensures all monthly paperwork is uploaded into the electrotonic health record. Ensures “ED to Go” content is updated every 6 months or more often as needed. Attends Interdisciplinary team meetings and staff meetings as scheduled. Acts as a responsible custodian for The Resource Center as the representative payee, and ensures the security of all assigned funds, including petty cash, personal fund expenditures, and peoples’ personal property, and secures itemized receipts for all funds spent. Uses person-centered language, adheres to professional boundaries, and promotes cooperation and teamwork, with primary focus to provide quality care to the people supported. Intervenes and reports any incidents or unusual occurrences as per policy/procedure. Ensures any immediate protective action is taken. Provides direct care to people as needed, within the scope of licensure. OTHER DUTIES: Maintains compliance with state and federal regulations as well as agency and program policies and procedures. Participates in quality assurance and site/agency re-certification process.
