The BH Access Counselor provides comprehensive diagnostic assessments and determines medically necessary service needs for clients. They also develop individualized service plans, coordinate care, and provide linkage to community resources to support recovery.

Summary

Provide comprehensive diagnostic assessment and determining medically necessary service needs of client. Clients not meeting criteria for a suspected behavioral health illness will be linked with appropriate services in the community. The goal for all clients is to meet their clinical needs, which will enable them to work towards recovery.

Qualifications/Skills

  • Bachelor’s in counseling, psychology, social work or another related field. Master’s degree in licensable field strongly preferred.
  • Must qualify as a MHP or higher per Illinois Department of Mental Health Rule 132
  • Illinois Certified Addiction Counselor (CADC), or licensed in Illinois as a LSW, LPC, or higher, within 2 years of employment.
  • Knowledge of DSM diagnoses and appropriate criteria for each diagnosis.
  • Knowledge of accepted assessment techniques
  • Ability to quickly establish rapport
  • Computer proficiency required,
  • Ability to navigate and complete documentation in the electronic healthcare record,
  • Ability to accurately complete required documentation within the prescribed time frames
  • Self-starter with ability to work independently,
  • Ability to work as a team member.
  • Valid driver’s license and ability to meet Agency insurance requirements (designated positions),
  • Must have own transportation (designated positions).

Essential Responsibilities

1. Work hours prescribed by the leadership and any additional hours deemed necessary for the responsibilities assigned.

2. Provide centralized diagnostic assessments that include computerized social histories, mental status, presenting problem, diagnosis, and treatment needs.

3. Determine appropriate level of care and treatment using centers utilization management criteria, develop corresponding ISP with clients, and coordinate needs with appropriate staff

4. Identify the need for immediate crisis intervention services

5. Maintain documentation per center quality assurance procedures and clinical performance standards

6. Assess client needs and strengths and establish a comprehensive plan that addresses support and treatment needs necessary for less restrictive care and stabilization to occur safely

7. Provide linkage, referrals and advocacy with community resources and community organizations

8. Cooperate with work team/center/on accomplishment of strategic objectives and activities

9. Ensure that the clinical treatment needs are met for consumers, which will enable them to work towards recovery.

10. Educate community agencies regarding services offered by Rosecrance

11. Serve as a member of the Access team and participate in all team meetings and activities.

12. Exercise confidentiality in keeping with the Code of Conduct and with the framework of the law.

13. Deliver exceptional customer service consistently to every customer.

Physical Requirements/Percentage of Work Time

1.

Vision: Must be able to read clinical documentation, policies and procedures and other written materials 75-100%

Hearing

Must be able to hear well enough to communicate with clients within an individual, group or family setting 75-100%; communicate with coworkers 25-49%; communicate on the telephone with a variety of individuals 25-49%

3. Standing/Walking/Mobility: Must have mobility between departments within the facility 25-49%

4. Climbing/Stooping/Kneeling: 0-24%

5. Lifting/Pulling/Pushing

Must exert up to 20 pounds of force occasionally and/or up to 5 pounds of force frequently to lift or move objects 25-49%

6. Fingering/Grasping/Feeling: Must be able to finger keyboard for computer work and phone equipment for placing/receiving calls 50-74%

7.

Sitting

must be able to sit for prolonged periods of time when conducting individual or group counseling, assessments, etc. 50-74%

Environment

Exposure to computers.

Exposure to bloodborne pathogens requiring the use of universal precautions and/or personal protective equipment.
