The Physical Therapist evaluates patients and develops individualized treatment plans to facilitate recovery and return to community activities. They collaborate with a multi-disciplinary team to provide care for patients with complex neurological conditions such as stroke and brain injury.
Evaluates all assigned patients and develops, implements and modifies treatment plan in accordance with the patient’s need and physician direction. Responsible for delegation and direction of all discipline-specific therapy services for those assigned patients. Ensures the design of functional and patient-centered treatment programs allowing for an effective return to community, vocational or academic activities. Serves as an educator of patients, families, students, co-workers and self. Performs other professional duties associated with the care of the patient, development of discipline-specific therapy services and design of program delivery. No new grads. Specific Duties: Evaluate the patient's level of functioning and develop an individualized treatment plan aligned with patient and family goals. Participates in discharge planning including, patient and family education, and recommendation of DME, community mobility as able. IPR- Inpatient rehabilitation Hospital- minimum of 2 weekend days per month are required/ 24 shifts per year minimum. Preferred Skills: Acute or IPR experience preferred Previous experience with the adult Neuro population
Current state licensure or eligibility for state licensure as a Physical Therapist required. Certified BLS or completion in the first 90 days of employment required.. #BSWIR - Inpatient Additional Data Equal Opportunity Employer including Disabled/Veterans
