About the Role

This position is a dual role that involves providing Peer Support and Substance Use Navigation services. Peer Support services will be provided to patients with substance abuse, mental health issues, and medical issues. Substance Use Navigator (SUN) services will be provided on-site at Yolo County jails/hospitals and in the local community. Peer Specialists are guided by a foundation of lived experiences in recovery and based on that experience, encourage, motivate, and support persons served and/or their families. Substance use navigator duties generally encompass connecting patients to evidenced based treatment modality and services. Travel and personal use of vehicle is required (mileage reimbursement rules apply). If you're passionate about making a real difference in your community, we want to hear from you.

Responsibilities

Provides information, education, and advocacy with technology to access other recovery services Maintains documentation in accordance with all Federal/State requirements as it applies to Peer Support role Maintains up to-date resource and referral information while developing and fostering relationships with referral sources. Connects patients to appropriate referrals and resources Supports collaboration and teamwork, which may include collaborating with other SUNs, social workers, nurses, discharge planners, and/or jail staff. Willingness to learn multiple Electronic Health Record systems in order to facilitate referrals. Functions as a positive, stable role model, demonstrating concern for patients. Attends clinical/medical team meetings as needed for patient support/advocacy. Provides patient advocacy and support for appointment service needs (child visits, court dates, medical/ psychiatric, and housing) Shares personal lived experiences related to behavioral health and recovery in a variety of settings including person-to-person, small and large group, and public presentations Attends and participate in committees and workgroups related to behavioral health, mental health, substance abuse, and related topics Participates in community activities related to Behavioral Health/ Substance Abuse Services. Performs related duties as assigned Assists patients in understanding how to cope with stigma Understands C.O.R.E’s written crisis policy and procedures Maintains documentation as required by all state, federal, county, and C.O.R.E. policies and regulations Facilitates educations groups as needed Maintains confidentiality in accordance with CFR 42, HIPPA, and Title 9

Requirements

Candidates for this position must be certified as a Peer Specialist in the State of California. Certified Specialist will be required to complete Federal or State required training and/or certification upon written notice from the appointing authority. The Peer Specialist must be able to work in various situations subject to defined policies, procedures, and practices. The person must have the ability to communicate clearly and concisely, both verbally and in writing. The person must possess and maintain a valid CA driver license (Class C or higher), fully registered vehicle, and insurance at all times. Lived experience with substance use recovery — strongly preferred and highly valued Knowledge of substance use disorders, recovery resources, and harm reduction principles Strong interpersonal and communication skills with a non-judgmental, trauma-informed approach Experience working in community health, social services, or a clinical environment preferred Ability to work collaboratively within a multidisciplinary care team About Us: C.O.R.E. Medical Clinic, Inc. is a Narcotic Treatment Program serving Yolo and Sacramento County with a commitment to compassionate care. This position is paid through Yolo County HHSA and is funded through June 30, 2027 but likely will continue beyond that or may be converted to a peer support specialist at C.O.R.E..
