Touchstone Health is seeking a full-time Hospice Social Worker in San Antonio who is called to walk alongside patients and families during some of life’s most meaningful and vulnerable moments. Key Responsibilities Provide social work services to hospice patients in accordance with the physician’s plan of care Support home health patients as needed, ensuring continuity of psychosocial care across settings Support patients and families in navigating emotional, social, and environmental challenges Connect patients to community resources, financial assistance, and support services Collaborate with the interdisciplinary team to develop and implement individualized care plans Assist with discharge planning and care transitions to ensure continuity and safety Document assessments, interventions, and care coordination accurately and in a timely manner
