The RN Case Manager will deliver skilled nursing visits and coordinate individualized care plans for patients in the OKC Metro area. They are responsible for documenting assessments, supervising home health aides, and collaborating with physicians and the interdisciplinary team.
Deliver skilled nursing visits to assigned patients based on physician-ordered plan of care Develop and regularly update individualized care plans Complete and document comprehensive assessments, including: Start of Care (SOC) Resumption of Care (ROC) Recertification Discharge assessments Supervise and support Home Health Aides Educate patients and caregivers on disease processes, medications, and rehab goals Attend and lead weekly case conferences for collaborative care planning Coordinate care with physicians and interdisciplinary team members Report any changes in patient condition to appropriate personnel promptly Ensure timely, complete, and accurate documentation that aligns with Medicare and agency guidelines Administer medications and treatments as prescribed Participate in on-call, holiday, and weekend rotations Assist with orientation and training of new staff Maintain compliance with all HIPAA and agency confidentiality standards Qualifications: Current RN license in the state of Oklahoma (Required) Minimum 1 year of Home Health experience (Preferred) Strong assessment and communication skills Experience with electronic medical records preferred Valid driver’s license, reliable transportation, and current auto insurance Ability to travel in varying weather conditions Must be able to travel to our main office in Norman, OK Schedule: Monday to Friday Flexible Shift On-call rotation once a month required Compensation & Benefits: Competitive salary Mileage reimbursement Health, Dental, and Vision Insurance Supplemental Benefits Paid Time Off (PTO) Flexible scheduling We’re looking for someone who is compassionate, self-motivated, and committed to delivering exceptional care.
