Position & Responsibilities

  • The Occupational Therapist (OT) provides occupational therapy services in compliance with AOTA standards, policies, and guidelines, applicable federal and state laws and regulations, and payer requirements related to occupational therapy practice and service delivery. Practices and maintain patient safety and dignity by abiding by Federal, State, Local and Optimal policy & procedures as pertains to patient protected health information and HIPAA (Health Information Portability and Accountability Act). The OT abides by the Occupational Therapy Code of Ethics. (AOTA, 2005) The OT abides by the Standards for Continuing Competence (AOTA, 2005) by maintaining and updating professional performance, knowledge, and skills. The OT is responsible for all aspects of Occupational Therapy service delivery and is accountable for delivery of safe, effective OT services. The OT respects the client’s sociocultural background and provides patient-centered and family-centered services ethically, with a best practice approach. The OT accepts and responds to referrals, directs the evaluation process utilizing defined protocols and standardized assessments, and analyzes and interprets the data in compliance with state and federal laws and other regulatory and payor requirements. The OT collaborates with the client(s) to evaluate the patient’s ability to participate in daily life with consideration of the patient’s history, goals, capacities, and needs and based on the activities the patient wants and needs to perform in the contexts and environments where the activities occur. The OT abides by the time frames, protocols, and standards established per home health care guidelines, federal and state law, and other regulatory and payor requirements. The OT communicates evaluation results within the established boundaries of patient confidentiality and privacy regulations (Health Insurance Portability and Accountability Act – HIPAA), by sharing information only with appropriate people involved in the client’s case. The OT recommends additional consultations by seeking MD orders and refers clients to appropriate resources when the needs of the client(s) will be best served by the expertise of other professionals or services. The OT has overall responsibility for development, documentation, and implementation of Occupational Therapy interventions based on skilled evaluation, patient goals, and clinical reasoning. The OT ensures that the Plan of Care is documented, communicated, and approved by the patient’s MD within the time frames formats, and standards established by the home health agency, state and federal laws, and other regulatory and payor requirements. The OT Plan of Care is developed and implemented in collaboration with the client(s) and other professional services, based on the patient’s needs and priorities, safety issues, and relative benefits vs risks of the proposed interventions. The OT uses clinical reasoning to select interventions including therapeutic use of self, therapeutic use of activities, consultation, education, and advocacy. The OT modifies the Plan of Care as needed throughout the episode and documents, communicates, and obtains updated MD orders to accommodate changes in the client’s needs, goals, and performance. The OT documents the OT interventions with the time frames, formats and standards established by home health and agency guidelines, federal and state law, and other regulatory and payor requirements. The OT communicates information effectively to appropriate personnel to facilitate continuity of care. The OT assists in coordinating care of all agency disciplines including, but not be limited to: Nursing, Certified Home Health Aide, Physical Therapy, Occupational Therapy, Speech Therapy, Medical Social Worker, Behavioral Health, and other services delivered to clients of the agency. The OT is responsible for selecting, measuring, interpreting, and documenting expected or achieved outcomes related to a patient’s ability to engage in activities. The OT is responsible for documenting changes in the patient’s performance and capacities for transitioning to other tasks or intensity of service or discontinuing services when the patient has achieved functional goals, reached the maximum potential for rehab, or wishes to discharge OT services. The OT develops and implements a transitional or discharge plan based on the patient’s needs, goals, performance, and access to beneficial community resources as indicated. The OT facilitates the transition or discharge process in collaboration with patients, their significant others and family, physicians, and other members of the interdisciplinary team. The OT is responsible for evaluating and documenting the safety and efficacy of occupational therapy interventions and outcomes in the patient’s home setting. When assigned to supervise a COTA, the OT completes a Supervisory Visit at least once every 30 days in compliance with home health regulations. Regulations do not require the COTA to be present during Supervisory Visits. When assigned to supervise a CNA, the OT completes an alternating schedule of CNA onsite Supervisory Visits and CNA Supervisory Visits without the CNA present every two weeks in compliance with home health regulations and agency protocol. The OT initiates cardio-pulmonary resuscitation and institutes other emergency procedures when necessary. The OT participates in Monthly, Quarterly, Annual trainings as well as Zone or other meetings required by Optimal. The OT complies with agency policies, protocols, and standard procedures. The OT completes all required documentation within 24 hours of the patient visit. The OT completes all documentation sent back for correction within 24 hours of receipt. The OT may be assigned other duties and tasks by the Optimal Home Care Emergency Command Center communication tree in the event of a local, regional or national emergency and/or disaster. The OT responds, as required, to support emergency response plan activation. The OT completes emergency preparedness training as identified as appropriate for this employee level. The OT participates in exercises and drills for emergency preparedness, as required. The OT completes other duties and tasks as assigned. The OT complies with agency Policies and Procedures as well as Employee Handbook detailing Human Resources policies. The OT completes other duties/responsibilities that support the agency’s core values of Commitment, Advocacy, Respect, and Excellence. The OT completes emergency preparedness training as identified as appropriate for this employee level. The OT participates in exercises and drills for emergency preparedness, as required. The OT performs other duties and tasks assigned by the Optimal Home Care Emergency Command Center communication tree in the event of a local, regional or national emergency and/or disaster.

Requirements

Qualifications

Graduate of an Occupational Therapy program accredited by the Accreditation Council for Occupational Therapy Education (ACOTE) or its predecessor organizations Must be currently licensed and in good standing in the state of Colorado. Must be able to communicate in the language of the client and document client related activity as appropriate. Possesses adequate and effective written and verbal communication skills. Possess adequate and effective interpersonal skills.
