About the role

The social worker provides psychosocial assessments, counseling, and supportive services to patients and families affected by illness or trauma. They are also responsible for managing program data, identifying funding sources, and facilitating program development.

Responsible for counseling, assessing, and assisting individuals/families or groups who are affected by illnesses or injuries, including rape, physical abuse, cancer, AIDS, and Alzheimer's, that may impair their ability to function normally. Principal Accountabilities: Location is Linden, NJ Provides psychosocial assessments to determine patient needs, makes appropriate referrals, provides counseling and supportive services to patients/families, makes referrals to the Division of Child Protection and Permanency (DCP&P) Identifies and maintains funding sources which supports the Family Counseling & Guidance Program. Maintains statistical information and analysis of data, develops outcome and measures for program evaluation, program development and facilitation of program. Other relevant duties as assigned

Qualifications

Required: High School Diploma or GED MSW from an accredited school of social work. NJ LSW or LCSW required. Previous medical hospital social work and grant experience strongly preferred.

Maintains current knowledge of trends and advances in clinical practice and healthcare informatics, as well as new developments and innovations in hardware and software technology. Demonstrated initiative, problem identification, resolution and analytical skills are essential, as well as excellent oral and written communication skills

Requirements

Candidates must hold a Master of Social Work (MSW) degree and possess a valid NJ LSW or LCSW license. Previous experience in medical hospital social work and grant management is strongly preferred.
