Provide routine daily nursing care and assistance with activities of daily living to residents in a skilled nursing facility. Document care accurately in medical records and report any changes in resident condition to nursing supervisors.
Provide routine daily care to residents, including assistance with activities of daily living (ADLs) Report all changes in resident condition to the Nurse Supervisor or Charge Nurse promptly Accurately document care provided on flow sheets, charts, and electronic medical records Measure and record vital signs (TPRs) as assigned Assist residents with bathing, grooming, dressing, toileting, and mobility Ensure residents’ comfort, safety, and dignity at all times Answer call lights promptly and respond to resident needs Assist with restorative and rehabilitative care as directed Maintain a clean, calm, and safe resident environment Work collaboratively with nursing staff and other departments Participate in shift reports and communicate effectively with the care team Perform other duties as assigned
Certificates, Licenses, Registrations Current, active California CNA certification BLS/CPR certification preferred Skills Strong communication and teamwork skills Compassionate, reliable, and professional demeanor Ability to follow directions and facility policies
