  • Summary: The Registered Nurse (RN) serves as a consultative clinical practice resource to Care Management teams supporting people with intellectual and developmental disabilities (IDD). The RN provides interdisciplinary consultation, clinical coaching, and risk/crisis mitigation support to strengthen care planning, promote safety, and improve health outcomes.

Duties and Responsibilities

  • Provide interdisciplinary clinical consultation and collaborative problem-solving support
  • Develop actionable clinical recommendations and action steps
  • Support crisis and risk mitigation through existing escalation pathways
  • Conduct targeted clinical record review
  • Complete timely clinical documentation in the EHR
  • Participate in follow-up and case closure activities Other duties: Adhere to all company policies and procedures. Follow all mandated reporting requirements. Perform other duties as assigned. Maintain confidentiality. Must possess a valid Driver’s License from New York, or a contiguous state (i.e., Connecticut, New Jersey, Pennsylvania, and Vermont) OR must have the ability to take ample public transportation to attend meetings in person in the community and in the office as needed. QualificationsEducation and Experience: Associates degree in Nursing and an active NYS RN license required. Minimum 3 years’ experience with either IDD population or an underserved population. Knowledge of community-based systems, hospital or acute care and of Medicaid, HCBS waivers, and managed care models preferred. Absolute sense of integrity and personal commitment to serving people with IDD and their families. Excellent interpersonal, public speaking, and written communication skills. Ability to work autonomously. Demonstrate leadership abilities. Demonstrate professionalism, respect, and ability to work in a team environment. Requirements
