About the role

The Memory Care Associate provides compassionate, person-centered care to residents with dementia, assisting with activities of daily living and mobility. They also monitor resident well-being, maintain a safe environment, and document changes in condition while fostering social engagement.

The P/T- The Memory Care Associate provides compassionate, person-centered care and support to residents living with dementia and other cognitive impairments. The Care Associate is responsible for assisting residents with activities of daily living while promoting dignity, independence, safety, and quality of life. The ideal candidate demonstrates patience, empathy, professionalism, and a genuine desire to build meaningful relationships with residents and their families. The Care Associate works as part of the Memory Care team to create a safe, welcoming, and engaging environment tailored to each resident's individual needs.

Resident Care & Support Provide respectful, compassionate assistance with activities of daily living, including bathing, dressing, grooming, toileting, continence care, and personal hygiene.

Assist residents with mobility, transfers, positioning, and ambulation according to their care plan and training.

Assist with meals and hydration while encouraging residents to maintain as much independence as possible.

Provide reminders and cueing as appropriate to help residents complete daily activities.

Follow each resident's individualized service plan and communicate changes in condition or needs to the appropriate supervisor.

Promote resident dignity, privacy, choice, independence, and personal preferences.

Memory Care Support Provide appropriate support to residents experiencing memory loss, confusion, anxiety, or behavioral changes.

Use effective dementia-care approaches, including redirection, reassurance, validation, and positive engagement.

Maintain a calm and supportive approach when residents become confused, upset, or resistant to care.

Observe residents for changes in behavior, mood, appetite, mobility, skin condition, or overall well-being and promptly report concerns.

Support residents in maintaining familiar routines and meaningful daily activities.

Safety & Environment Maintain a safe, clean, organized, and comfortable resident environment.

Follow all community safety procedures, emergency procedures, infection-control practices, and fall-prevention protocols.

Monitor residents and report safety concerns, accidents, incidents, or unusual behaviors immediately.

Follow appropriate procedures regarding wandering, elopement risk, and other safety concerns.

Maintain confidentiality of resident and employee information.

Documentation & Communication Complete required documentation accurately and timely.

Communicate resident needs, concerns, and changes in condition to the Memory Care Director, nurse, or appropriate supervisor.

Participate in shift-to-shift communication and resident care discussions.

Report incidents, injuries, medication concerns, or changes in resident condition according to community policy.

Maintain professional communication with residents, families, coworkers, and leadership.

Activities & Engagement Encourage residents to participate in meaningful activities, social programs, exercise, dining, and community events.

Assist with activities designed to promote cognitive stimulation, social connection, independence, and emotional well-being.

Engage residents in conversation and activities based on their individual interests, abilities, and life experiences.

Help create a positive and welcoming atmosphere within the Memory Care neighborhood.

Requirements

Qualifications

High school diploma or equivalent preferred. Previous experience working with older adults, individuals with dementia, or in a senior living, healthcare, or caregiving environment preferred. Must demonstrate the ability to communicate effectively with residents, families, and team members. Must be compassionate, dependable, patient, and able to work effectively as part of a team. Must be able to follow written and verbal instructions and accurately document resident care. Must successfully complete all required background checks, screenings, training, and community onboarding requirements. Physical Requirements The employee must be able to perform the essential functions of the position with or without reasonable accommodation. Responsibilities may include: Standing and walking for extended periods. Bending, stooping, reaching, and kneeling. Assisting residents with transfers, mobility, and activities of daily living. Lifting and/or assisting with movement of residents and equipment in accordance with established safety procedures. Responding promptly to resident needs and emergency situations.
