Licensed Practical Nurse (LPN)

Position Summary

The Licensed Practical Nurse (LPN) functions as an integral member of the Primary Care Team, providing clinical support and direct patient care under the supervision of a registered nurse and/or primary care provider. The LPN assists in delivering comprehensive healthcare services, promotes preventive care and wellness, supports chronic disease management, and helps ensure efficient clinic operations. License Practical Nurse (LPN) Job Responsibilities:

Assist providers with patient examinations, procedures, and treatments.

Perform patient intake, including obtaining vital signs, medical histories, medication reconciliation, and documenting patient concerns.

Provide health screenings and routine clinical assessments within scope of practice.

Administer medications, immunizations, and treatments as ordered by the provider and in accordance with agency policies.

Perform phlebotomy and specimen collection procedures for which the LPN is trained.

Provide patient education regarding preventive health, chronic disease management, medications, treatment plans, and healthy lifestyle choices.

Support care coordination by communicating with patients, families, pharmacies, specialists, and community resources as appropriate.

Assist with medication refills, prior authorizations, and referrals to specialty care providers.

Requirements

Candidates must be graduates of an accredited LPN program and hold a current New York State LPN license in good standing. At least one year of clinical nursing experience is preferred, along with current BLS certification and proficiency in EMR systems.

Proficiency in utilizing electronic medical records (EMR) systems.

Current BLS certification required.

Valid New York State driver's license and reliable transportation when required.
