The Resource Navigator conducts comprehensive assessments to identify patient needs and develops individualized action plans to overcome barriers to healthcare and social services. They also coordinate follow-up support, facilitate public benefit applications, and maintain collaborative relationships with community partners.

Description The Resource Navigator supports St.

Petersburg Free Clinic’s (SPFC) mission by helping individuals overcome barriers to healthcare and essential community services.

The Resource Navigator works one-on-one with patients, clients, and community members to address both medical and non-medical needs, recognizing that health outcomes are shaped by a broad range of social and economic factors.

Through assessment, resource coordination, and follow-up support, the Resource Navigator helps individuals access healthcare services, public benefits, food assistance, housing resources, transportation, and other community supports.

The Resource Navigator also contributes to the development and maintenance of SPFC’s resource network and serves as a knowledgeable resource for staff and volunteers.

This position reports to the Director of Navigation.

Supervisory Responsibilities None Position Responsibilities Contribute to an environment of respect and dignity through every interaction with patients, clients, community members, volunteers, and co-workers.

Conduct comprehensive assessments with patients, clients, and community members to identify healthcare, social service, and resource needs and develop individualized action plans to address barriers to care and stability.

Provide direct navigation support to individuals seeking healthcare, specialty medical services, public benefits, and community resources, ensuring timely and appropriate connections to services.

Coordinate ongoing follow-up with patients and clients to ensure successful access to care, completion of referrals, and connection to community resources.

Collaborate with fellow navigators and other SPFC departments to provide coordinated, person-centered services and ensure individuals are connected to the full range of resources available through SPFC.

Facilitate applications and enrollment for healthcare coverage, financial assistance programs, public benefits, and other community resources, including maintaining Certified Application Counselor (CAC) Certification and training requirements.

Assist patients and clients with SNAP and other public benefits applications and maintain collaborative relationships with benefits partners.

Maintain current knowledge of healthcare systems, public assistance programs, eligibility requirements, and community resources available throughout Pinellas County.

Develop and maintain collaborative relationships with healthcare providers, community organizations, government agencies, and other service providers to best serve client needs.

Maintain accurate and timely documentation, client records, referrals, outcomes, and program data in accordance with agency, funding, and regulatory requirements.

Identify and implement opportunities for community resources to reach patients and clients on-site.

Maintain relationships with partner agencies.

In partnership with the Director of Navigation, participate in and represent SPFC at approved community events, outreach activities, networking meetings, and partner convenings to increase awareness of services, strengthen community relationships, and connect individuals to available resources.

Assist in developing and maintaining resource guides, referral materials, and resource databases to support staff, volunteers, patients, clients, and community members.

Educate other staff and volunteers about relevant services available to our clients.

Requirements

Agency-wide Responsibilities Maintains documentation and records in compliance with HIPAA, and SPFC policies and procedures. Maintains confidentiality. Ensures communication and interactions with patients, clients, and community members are conducted with dignity and respect. Adheres to agency policies and procedures and all Federal and State regulations and local ordinances. Required Skills & Abilities Strong relationship-building skills with both internal and external stakeholders. Excellent communication and client engagement abilities. Strong problem-solving, critical thinking, and resource coordination skills. Ability to work effectively across departments and with community partners. Demonstrated ability to manage multiple priorities and follow through on client needs. Experience in maintaining accurate records and databases. Proficient in Microsoft Office Suite and related software. Ability to learn and effectively utilize a variety of technology platforms and software. Commitment to SPFC’s mission and values, and a person-centered approach to service. Ability to meet deadlines. Knowledge of healthcare systems, public health benefits, and community resources preferred. Experience in conducting community outreach, engagement, or resource navigation preferred. Education & Experience: Bachelor’s degree in Social Work, Human Services, Public Health, Healthcare Administration, or a related field preferred; or equivalent work experience. Minimum of two years of experience in social services, healthcare navigation, case management, or a related field. Knowledge of public assistance programs, healthcare coverage options, and community resources available throughout Pinellas County preferred. Bilingual (Spanish) strongly preferred.

Physical Requirements Prolonged periods of sitting at a desk and working on a computer. Must be able to lift up to 15 pounds at times. SPFC is a Drug and Alcohol-Free Workplace.
