This position is responsible for planning, coordinating, providing and documenting care for terminally ill patients from day of admission through day of discharge.

Utilizing physicians' orders and professional skills, and in coordination with the other members of the interdisciplinary team, the Case Manager develops, coordinates and implements a hospice plan of care that meets each patient's specific needs and is in compliance with Federal and State regulations, reimbursement guidelines, as well as the organization's policies and procedures Your Responsibilities (Includes but Not Limited to): Provide skilled nursing assessment and planning and coordination/care management across settings to patients in private homes, adult foster homes, residential/assisted living care, and/or nursing facilities.

Maintain up-to-date patient records, including recording problems, plans, actions, and goals accurately and clearly reflective of changes as they occur in accordance with policies and procedures.

Provide nursing care, attentive to physical, emotional, and spiritual needs of patients.

Counsel patients and families regarding symptom control, medications, direct nursing care measures, nutrition, etc.

Assist patients and families in making choices that best meet their needs by providing alternatives and support.

Develop and implement a plan of care for patient/family/ and/or caregivers in consultation with other interdisciplinary group members (IDT).

Communicate often and well with other Care Partners staff; function as an integral member of the patient care team.

Make visits at death, support families, assist with final arrangements.

Educate families, arrange services, order equipment, obtain physician orders, and order medications.

Collaborate professionally with other health care providers, including the patient's physician, pharmacies, and care facilities to ensure optimal care for patient.
