We provide each person entering our program their own apartment, without requiring treatment compliance or sobriety as a prerequisite. We then provide each person with intensive support services to help them stay housed and begin their journey to recovery.
The Assertive Community Treatment (ACT) team provides flexible, community-based services that are designed to promote the stability, recovery and community reintegration of formerly homeless individuals who have a mental illness and co-occurring substance use disorders.
The Service Coordinator works in collaboration with the trans-disciplinary team (including Psychiatrist, nurses, addictions counselors, and peers) and provides ACT services for all clients on the team in a compassionate and responsible manner. All services consumers are consumer-driven and recovery-focused.
Successful Service Coordinators are flexible and patient. They work well in a fast-paced environment, and are independent thinkers.
Candidates must hold a bachelor's degree in social work or a related field, or possess two years of experience in mental health or human services. A valid driver's license and personal vehicle are required for community outreach duties.
