We are seeking a Registered Nurse (RN) to join our Home Health Team in Sarasota County. In this role, there is a real impact in the comfort of patients’ homes. As part of our compassionate, high‑performing care team, you’ll provide skilled, patient-centered home health care through assessments, care planning, clinical interventions, patient education, and collaboration with the care team while maintaining accurate documentation and compliance with regulatory standards.

How will you make an impact & Requirements
Conduct comprehensive assessments of patients' physical, mental, and emotional health, including vital signs, medication administration, wound care, and disease management.
Care
Planning: Develop individualized care plans that address patients' specific needs and goals, including medication administration, wound care, and disease management.
Patient
Education: Educate patients and their families on disease management, medication administration, and self-care techniques to promote independence and improve health outcomes.
Clinical
Interventions: Provide direct patient care, including wound care, medication administration, injections, and IV therapy.
Collaboration: Collaborate with physicians, therapists, and other healthcare professionals to ensure coordinated and comprehensive care.
Documentation: Maintain accurate and timely documentation of patient assessments, interventions, and progress notes.
Compliance: Adhere to all relevant regulatory standards, including HIPAA and Medicare regulations.
Current and valid Registered Nurse (RN) license in the state of Florida.
Experience: Minimum of 1 year of experience in a hospital or skilled nursing facility.
Skills: Strong clinical skills, excellent communication and interpersonal abilities, and a patient-centered approach to care.
Professionalism: Strong work ethic, reliability, and a commitment to providing high-quality care.
Transportation: Reliable transportation to visit patients in their homes.
