The Neurology Nurse Practitioner or Physician Assistant will develop and implement individualized treatment plans for patients with neurological conditions. They will also conduct follow-up visits, coordinate care with other healthcare professionals, and provide education to patients and their families.
Develop and implement individualized treatment plans for patients, including medication management and non-pharmacologic interventions Conduct regular follow-up visits to monitor patient progress, adjust treatment plans as necessary, and provide education and support Collaborate with physicians and other healthcare professionals to coordinate care and ensure a multidisciplinary approach Provide patient and family education on disease management, medications, and lifestyle modifications Maintain accurate and timely documentation of patient encounters and treatment plans Participate in quality improvement initiatives to enhance patient outcomes and optimize patient care delivery Perform comprehensive assessments to accurately diagnose and treat patients with neurological conditions
Minimum 6 months of experience in long-term care or a skilled nursing facility Active and unrestricted Nurse Practitioner, Physician Assistant, or Physician license DEA License CPR BCLS/BLS certified Must hold a Master's Degree in Nursing Completed a degree from an accredited Nurse Practitioner program A year of experience in Neurology Preferred experience in Documenting EMR (Sigma, Visual, PCC, etc.): training provided Must have great organizational and time management skills. Must be able to lift a minimum of 20 pounds and stand / walk / work on your feet up to 6-8 hours per day
