Responsibilities
Conduct comprehensive in-home patient assessments to determine care needs and eligibility for home health services Complete OASIS assessments, start-of-care evaluations, and recertification assessments in compliance with regulatory standards Collaborate with physicians, care teams, and family members to develop and communicate individualized care plans Document clinical findings accurately and in a timely manner using electronic health records (EHR) Ensure compliance with Medicare, Medicaid, and Joint Commission guidelines during all assessment activities Communicate patient status changes and clinical concerns to the appropriate care team members Maintain a flexible schedule to accommodate patient and agency assessment needs across the Arlington, VA service area
Requirements
