We invite experienced Nurse Practitioners and Physician Assistants to discover all that we can do when we bring healing hearts, inquisitive minds and progressive visionaries together within our Atrium Health Transition Services Team in the greater Charlotte area.

Atrium Health Transition Services Clinics are outpatient post-discharge clinics, committed to helping our patients restore their body and mind post hospitalization.

Our Transition Clinic is a multidisciplinary ambulatory Internal Medicine clinic for patients at high risk of complications and readmissions due to medical complexity or barriers to healthcare access.

We offer integrated visits with provider, pharmacist, and social worker.

During our 30 day follow up period, we identify and alleviate barriers to health focusing on patient engagement, access, and education.

Care is delivered in office, virtually and/or with community paramedicine support based on patient’s needs.

Our team is committed to providing the highest quality of medical care to all patients and addressing barriers to health Highlights about the position include: 100% Outpatient Join current team of 3 Physicians & 2 APPs Office hours Monday-Friday (Flex (8)am-5pm) Call rotation: Shared coverage rotated a week at a time; phone only No Nights/Weekends Work at Transition clinic locations in greater Charlotte and provide virtual care.

Requirements

Candidates must be experienced Nurse Practitioners or Physician Assistants with an interest in internal medicine. Experience in managing complex medical patients and working within a multidisciplinary team is preferred.
