The RN Admissions Nurse position is a full-time 10-hour shift position that has a work schedule from 12:00PM to 10:30PM. This position requires the candidate to work every other weekend and some holidays.

  • SUMMARY: The RN Admission Nurse works under the general direction of the Patient Care Manager who oversees the admission of patients to hospice care services. The RN Admission Nurse provides informed consent for hospice services, performs the initial admission assessment, is responsible for the initial ordering of medications and supplies, while reinforcing the education patient/family has received regarding the scope of the hospice care and the role of the hospice care team members. When not performing admissions, the RN Admission Nurse works with the other teams to fulfill a variety of nursing needs for current patients/families enrolled in hospice care services.

Responsibilities

Responsible for providing informed consent for hospice services, nursing assessment and initial care planning prior to and during the hospice admission process. Responsible for evaluation of patient condition, assessing patient current condition and recent history to assist team in evaluating appropriateness for hospice services. Responsible for triage calls/visits when no admissions are assigned during scheduled work hours. Maintains regular communication with the Patient Care Manager and keeps themselves informed of any unusual or problematic patient/family issues effecting the admission process and ongoing patient care. Initiate communication with attending physicians, other hospice team members and other community agencies as needed to coordinate optimal care and use of resources for patient/family. Completes accurate and timely documentation of the initial assessment and plan of care. Perform scheduled home visits to assess needs; teach caregivers pain management, symptom control and patient care skills; and provide physical, emotional and spiritual support to the patient/family. Attend death events and provide appropriate support at time of death. Participate in CHHS orientation and supervision of students. Attend nursing department meetings, All Staff meetings, and required in-services. Follows all CHHS policies and procedures including, but not limited to, safety, infection control, privacy, confidentiality, and HIPAA. Maintain RN license in accordance with federal, state and local laws, and must act only within the scope of his or her state license. All personnel qualifications must be kept current at all times. Other Functions: Maintain a professional appearance of self and working area; exhibit appropriate behavior with guests and clients, i.e., courteous, thoughtful and professional at all times. Other duties as assigned, including work in other areas to cover absences or relief to equalize peak work periods or otherwise meet the needs of the organization. Responsible for establishing and maintaining a safe environment for patients/families, and a safe working environment for self and coworkers. Performs job duties and interacts with patients/families, CHHS staff, and community members in a courteous and professional manner. Maintains all required confidentiality and privacy requirements of both the organization and the regulatory agencies that have oversight of and for CHHS. SUCCESS FACTORS: Ability to be flexible. High degree of clinical competence. Able to instill confidence and calm in patients and families on short acquaintance. Excellent communication skills. Especially empathic listening. Attention to detail. Excellent assessment and documentation skills. Qualifications Graduation from an accredited school of nursing. At least one year of current nursing experience (LVN or RN). Valid California RN license. Current California driver’s license and automobile insurance required. Certified Hospice and Palliative Nurse (CHPN) certificate is preferred. Possesses and maintains current CPR certification. WORK ENVIRONMENT: The main environment for this position is in the patients' homes, which vary in levels of cleanliness, accessibility, and climate control. The employee may be exposed to virus, disease and infection from patients. The employee may experience situations, with emotionally disturbed, substance abuse, serious illness, and deceased patients. While performing the duties of this position, the employee will travel by automobile and is exposed to changing weather conditions.

Requirements

Candidates must hold a valid California RN license and have at least one year of current nursing experience. Graduation from an accredited school of nursing and current CPR certification are required.
