The Program Community Psychiatric Rehabilitation Center (CPRC) supports adults and children with disabilities in social role functioning and daily living skills. Staff work closely with clients in various community settings, providing case management services and therapy to promote recovery.

The Job Responsible for delivering community support services for assigned clients within the CPRC. The role involves working in the community, such as client homes or residential centers. Typically, the position manages 20-25 clients, meeting with them weekly.
Essential Functions 1.
Provide community support services to children and adults enrolled in the CPRC program.
2.
Maintain a caseload, including community and hospital settings.
3.
Provide training/education to clients in daily living skills.
4.
Monitor treatment plan goals and maintain contact with the treatment team.
5.
Refer clients to appropriate community resources to improve functionality and develop a support system.
6.
Document client interactions per agency guidelines, completing all documentation by deadlines according to clinical policies, DMH, CARF, and federal/state program standards.
