  • Shift: First ShiftThe Social Services Assistant supports the Social Services Department in providing essential services to residents or patients in a healthcare, long-term care, or assisted living facility. The role involves helping residents and their families address psychosocial, emotional, and practical concerns, as well as assisting with discharge planning, community resource referrals, and care coordination. The Social Services Assistant works closely with the Social Services Director, healthcare professionals, and residents to ensure that residents' overall well-being and quality of life are maintained.
