CNA (PRN) - Hospice

Preferred Requirements Preferred Education High School Diploma or GED Equivalent Preferred Experience N/A Preferred License/Registration/Certifications N/A Core Job Responsibilities Adheres to the care plan established by the RN or Therapist; assists patient/family as needed.

Notifies RN or Therapist when updates to care plan are needed.

Assists patients with personal care activities including but not limited to: bathing, dressing, grooming, skin care, hair care, nail care, feeding, oral hygiene, and shaving.

Toilet's patients and assists with bedpans and urinals; provide catheter care, empty catheter bag, if applicable.

Assists patients with use of cane or walker, special eating utensils, special techniques, and equipment for personal hygiene.

Document's care performed as outlined on care plan in the electronic medical record.

Reports all changes in patient's condition (physical and/or mental) and environmental changes to case manager.

Assists family members with and provides training on lifting, turning, re-positioning, and transferring patient.

Provides emotional support to patients and/or family members.

Maintains a safe, secure, and healthy patient environment by following asepsis standards and policies.

Participates in 12 hours of in-service training.

Performs other job duties as assigned.
