Occupational Therapist
The Occupational Therapist provides skilled therapy services in patient homes, including evaluations, treatment planning, and the implementation of individualized care plans. The Occupational Therapist (OT) provides skilled occupational therapy services to patients in their homes in accordance with physician orders, the plan of care, agency policies, and applicable state and federal regulations.
The OT evaluates patient functional status, develops and implements therapy interventions, and works collaboratively with the interdisciplinary care team to improve patient independence, safety, and quality of life.
This position reports to the Director of Nursing (DON), Clinical Manager, or designated clinical leadership. The OT functions as part of the interdisciplinary care team and provides supervision to the Occupational Therapy Assistant (OTA) and Home Health Aide when required by the patient’s plan of care and agency policy.
Scope of Practice
The Occupational Therapist performs services within the scope of Florida OT licensure, Medicare Conditions of Participation, physician orders, and agency policies. The OT is responsible for evaluation, treatment planning, reassessment, and therapy supervision within the home health setting.
Work is performed in patient homes and community settings with exposure to variable environmental conditions, including pets, smoke, infectious diseases, and weather-related travel conditions.

Candidates must be graduates of an accredited Occupational Therapy program and hold a current state OTR license. A minimum of one year of experience is required, along with a valid driver's license and current CPR certification.
