JOB DESCRIPTION SUMMARY Responsible as a member of the interdisciplinary team and to the attending physician/hospice physician for the provision of skilled nursing assessment, planning, and care to maximize the comfort and health of patients and families consistent with HOA policies and procedures. Responsible for educating patients and families regarding end-of-life procedures and hospice services. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Provides information to those individuals, families, and physicians inquiring about hospice services. Takes referral information from families, doctors, hospitals and other agencies. Does an initial evaluation either alone or with a hospice physician within 24-48 hours of referral or per family request. Coordinates DME, pharmacies, other agencies involved when needed by patient/family. Provides complete information to the admission director and team leaders to facilitate admission. Completes required admission documentation. Coordinates assignment of RN and HHA with appropriate personnel. Follows up with all therapies begun during initial evaluation. Completes written assignment for RN and HHA and submits in a timely manner. Responsible for managing patient/family concerns and issues. Develops individualize plan of care based on the needs of patient. Report to personnel involved in care on day of admission. Documents instructions given to the patient/family and prepares patient/caregiver for self-care. Documents patient/family responses to teaching and learning. Assists with in-services to hospital and other agencies as assigned. Attends required meeting and educational sessions. Participates in on-call duties & holiday shift as defined by the on-call schedule/policy when required. RN will be placed on pull-list (log) that serves as potential coverage for after-hour shifts that are vacant due to employee using paid time-off. The above statements are intended to be a representative summary of the major duties and responsibilities performed by incumbents of this job. The incumbents may be requested to perform job-related tasks or special projects, other than those stated in this description, by supervisors/Leadership Team.

  • Licensure/Certification/Registration: Currently licensed in Louisiana as a Registered Nurse.

Education

Graduate of an accredited nursing school.

Experience

  • RN-three (3) years of experience; preferred, one full year of hospice experience.
  • Special
  • Qualifications: Understands hospice philosophy and has knowledge of CMS hospice certification process. Must have experience with issues concerning the terminally ill and death/dying. Should have good oral, written and organizational skills. Ability to use computer, calculator, and a wide variety of clinical equipment. Possesses current Louisiana Driver's License, Automobile Liability Coverage, and CPR cert.

Requirements

Candidates must be a Registered Nurse licensed in Louisiana with at least three years of nursing experience, preferably including one year in hospice. A current driver's license, automobile liability coverage, and CPR certification are also required.
