Our Values R – Respect I – Innovation S – Stewardship E – Excellence CDR Outpatient Therapist Summary The CDR Outpatient Therapist is a part-time position under the supervision of the CDR Program Manager.

The principal responsibilities of this position are to provide counseling for the population served.

This person may diagnose and treat emotional and behavioral problems under supervision specified by licensure.

This person is responsible for the clinical record and collaboration with other providers to provide comprehensive and client-focused care.

A Day in the Life · Provide individual and/or group counseling to population served and developing a therapeutic alliance with clients by conveying sensitivity through words and tone of voice to establish an environment of change utilizing evidenced based and/or research-based treatment interventions, and following agency guidelines regarding utilization management · Provide ongoing assessment of clients, which includes diagnosis, mental status, level of functioning, and client risk to self and others. · Utilize techniques consistent with standards in the field and supported by clinical research, using evidence-based and/or research-based practices established by the agency, consistent with ethical/procedural standards and within the scope of practice as specified by education and licensure. · Work collaboratively with clients to define specific, observable and behavioral treatment goals that are defined in the positive and include a baseline. · Work collaboratively with clients to agree on tasks that match client’s readiness for change · Meet productivity or contracted billable services per fiscal year, 100% of agency standard per fiscal year.

Clients will be scheduled utilizing available hours provided by the clinician. · Maintain all Clinical Record documentation that meets all agency standards and timelines as measured by Clinical Records and Peer Review audits, supervisory audits and Financial Assessment Updates.

Achieve an annual average of at least 80% on initial score of the clinical records review. · Complete progress notes at time of service. · Administer outcome ratings on all primary cases based on outcome schedule and appropriately link outcome ratings to treatment goals. · Administer Client Satisfaction Surveys to all primary cases, annually. · Ability to document best efforts to re-engage clients before termination and collaborate in cases of all transfers and referrals.

Completes terminations for all clients who have been inactive for periods not to exceed requirements indicated in the Clinical Records Manual. · Work collaboratively with clients and other caregivers to actively engage family members and other community supports in clients’ recovery. · Provide community psychiatric support to primary and secondary cases as clinically indicated. · Participate in team meetings, and case conferences as it relates to team and client care. · Consult with PHS and external providers as needed to advocate for the client and to facilitate progress during counseling. · Maintain license and/or registration during active employment appropriates to profession and job requirement as specified under minimum qualifications. · Performs all other duties and tasks as assigned.

Requirements

Success Requirements To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Education/Experience A Master’s degree in Counseling, Social Work, or related field from an accredited College or University, plus two (2) years clinically supervised experience. Language Skills Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of organization. Reasoning Ability Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form. Computer Skills To perform this job successfully, an individual should have the ability to gain knowledge of current practice management system, electronic medical record, Microsoft Word, text paging, Internet, and Intranet. Certificates, Licenses, Registrations Holds license through The Ohio Counselor, Social Worker, and Marriage and Family Therapist Board, which qualifies the employee to provide counseling and psychotherapy services. Must earn and maintain American Heart Association or American Red Cross, Basic Life Support (BLS) certification, training can be provided through PHS. Specific vision abilities required by this job include close vision, distance vision, peripheral vision, depth perception and ability to adjust focus. Work Environment The work environment characteristics described here are representative of those an employee encounters while performing the essential functions of this job.
