The Case Manager coordinates client care, facilitates the intake process, and develops individualized treatment plans to support recovery. They also serve as an advocate for clients while ensuring compliance with program requirements and maintaining accurate documentation.

The Case Manager is responsible for coordinating client care, facilitating the intake and admission process, developing treatment plans, and assisting clients with resources needed to support recovery and successful treatment outcomes. The Case Manager serves as an advocate for clients while ensuring compliance with program requirements and documentation standards.

Key Responsibilities

Conduct client intakes, admissions, and orientation to program services. Complete biopsychosocial assessments and gather required admission documentation. Coordinate care with medical providers, therapists, family members, referral sources, and outside agencies. Develop, implement, and monitor individualized case management plans. Assist clients with housing, employment, transportation, insurance, benefits, legal obligations, and community resources. Schedule and coordinate medical, psychiatric, and other supportive appointments. Maintain accurate and timely documentation in the electronic health record. Monitor client progress and address barriers to treatment and recovery. Participate in multidisciplinary team meetings and treatment planning. Facilitate discharge planning and aftercare coordination. Ensure compliance with confidentiality regulations and program policies.

Qualifications

Proficient in the intake and admission process. Strong case management and care coordination skills. Knowledge of substance use disorder and/or mental health treatment services. Experience with electronic health records (Kipu preferred). Excellent communication, organization, and documentation skills. Ability to work effectively with clients, families, treatment teams, and community partners. CADC or related certification preferred. CPR/First Aid certification preferred. Skills & Competencies: Client advocacy and engagement Crisis intervention and problem-solving Resource coordination and referral management Time management and organizational skills Professional documentation and record keeping Ability to maintain confidentiality and professional boundaries This position plays a vital role in helping clients navigate treatment, remove barriers to recovery, and achieve successful discharge outcomes.

Requirements

Candidates should have strong case management skills and knowledge of substance use or mental health treatment services. Proficiency with electronic health records and relevant certifications like CADC or CPR/First Aid are preferred.
