About the role

The Licensed Social Worker will conduct comprehensive psychosocial assessments and develop individualized plans of care for hospice patients and their families. They will also provide emotional support, crisis intervention, and coordinate community resources while collaborating with the interdisciplinary hospice team.

Licensed Social Worker (Hospice) – Continuum Care Hospice Continuum Care Hospice is seeking a compassionate, dedicated, and experienced Licensed Social Worker (LSW) to join our interdisciplinary hospice team.

This is a PRN position.

The Licensed Social Worker will assist in planning, organizing, implementing, evaluating, and directing the overall operation of social services for hospice patients and their families in accordance with current federal, state, and local regulations, as well as Continuum Care Hospice policies and procedures.

The ideal candidate is committed to providing patient-centered care, emotional support, advocacy, and resource coordination to individuals facing life-limiting illnesses and their loved ones.

Duties and Responsibilities

Conduct comprehensive psychosocial assessments of patients and families. Develop and implement individualized plans of care addressing psychosocial, emotional, and environmental needs. Provide counseling, emotional support, crisis intervention, and bereavement support to patients and families. Assist patients and families with community resources, financial assistance programs, advance directives, and healthcare decision-making. Collaborate with physicians, nurses, chaplains, aides, and other interdisciplinary team members to ensure coordinated care. Participate in care planning meetings and maintain accurate clinical documentation. Advocate for patient rights and ensure compliance with hospice regulations and standards. Assist with discharge planning and transitions of care when appropriate. Maintain confidentiality and professionalism in all patient interactions. Education and Experience Required Qualifications Master's Degree in Social Work (MSW) from an accredited School of Social Work. Current and active Licensed Social Worker (LSW) license in good standing. Minimum of two (2) years of supervised social work experience in a healthcare setting working directly with individuals. Knowledge of psychosocial principles, community resources, and healthcare systems. Strong communication, organizational, and interpersonal skills. Ability to work independently and collaboratively within an interdisciplinary team.

Preferred Qualifications

Previous hospice, palliative care, home health, or long-term care experience. Knowledge of end-of-life care practices and bereavement support services.

Requirements
