Job Description Saint Luke's East Hospital is seeking a licensed Social Worker to join our team!
The Opportunity: Responsible and accountable for assessment of psychological, social, emotional, cultural and financial needs, and the resulting treatment that impacts patients' health and recovery.
Provides professional social work services that utilize clinical knowledge to address the needs of patients and families during illness, crisis intervention, and post-acute discharge planning.
Collaborates with multidisciplinary team to develop transition plan and address needs in a timely manner.
Participates in program planning and development to meet the needs of customers within the entity, health system and community.
Candidates must hold a Master's degree in Social Work and possess a valid LMSW or LCSW license in Missouri. At least one year of applicable experience is required, with preference given to those with a background in a healthcare setting.
