Serves as a clinical expert through patient education, interdisciplinary collaboration, and consultation, while maintaining program excellence and professional growth. Responsible for developing comprehensive care plans, coordinating patient care, delivering ongoing treatment while fostering a strong patient-provider relationship. Committed to cost-effective care management, optimizing resource utilization, and enhancing patient experiences through continuous process improvement. Provides direct patient care while collaborating with a multidisciplinary healthcare team to ensure high-quality, evidence-based outcomes.

Job Description

Education: Required: Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited school of nursing

Licensure/Certification

  • Required: Active and unrestricted CA licensure as Registered Nurse (RN) and Nurse Practitioner (NP); Nurse Practitioner Furnishing (NPF) license number from the California Board of Registered Nursing (BRN); Current board certification as NP by a nationally accredited organization in a population foci; Drug Enforcement Administration (DEA) certificate to dispense controlled substances (II-V); National Provider Identifier (NPI); Current Basic Life Support certification (BLS) from the American Heart Association (AHA); Advanced Cardiovascular Life Support (ACLS) from AHA required upon hire and Adult Acute Care or Adult Gerontological Acute Care board certification required by December 31, 2028 if assigned to
