Client Care Coordinator

The Client Care Coordinator (MSW) is responsible for improving access to mental health services in the shelter and for assisting families that are homeless with navigating multiple systems and coping with the stressors and anxiety induced by the homelessness.
The essential functions of the job include, but are not limited to the duties listed in the job description.
Essential job functions: Through the employee’s own efforts, the employee accomplishes the following essential functions:
1.
Completes a comprehensive biopsychosocial assessment with each family to understand strengths and service needs.
2.
Provide short-term counseling, crisis intervention, risk assessment, safety planning, and psychoeducation for families.
3.
Collaborates with ACS and/or preventive services agency when a family is child welfare involved; participates in ACS conferences to advocate for the family.
4.
Makes referrals and appropriate linkages.
5.
Facilitates group work (e.g. with young people, with fathers).
6.
Communicates and collaborates with service providers to enhance engagement and ensure quality.
7.
Provides education/training to other staff.
8.
Facilitates quarterly family meetings (with the family and shelter staff) to discuss planning efforts and progress.

To perform this job successfully, and individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skills, and / or ability required.
Ascending or descending stairs and ramps using feet and legs and/or hands and arms. Must be able to walk up and down stairs (about 10 flights) in order to patrol stairways, respond to security incidents, and assist in the evacuation of clients during an emergency. Body agility is emphasized. The amount of climbing required exceeds that required for ordinary locomotion.
