About the role

The pharmacist is responsible for supervising and performing the compounding, packaging, and dispensing of pharmaceuticals in compliance with state and federal regulations. They also monitor patient profiles to ensure appropriate dosage and therapy while providing evidence-based clinical pharmacy care.

Newport News, Virginia Hiring Range $54.80 - $79.48/Hourly Actual pay is determined based on job-related factors such as relevant experience, education, credentials, skills, internal equity, and business needs. Schedule - Monday - Friday 3pm - 11:30pm

Overview Supervises and performs compounding packaging and dispensing of pharmaceuticals according to prescription, physician order or floor stock request in compliance with state and federal law and regulation. Possesses the skills and ability to assess data reflective of patient status and to interpret this information and identify the patients' needs in relation to age specific requirements, and to provide care as described in accordance with state and federal law and regulation, and facility policy and procedures. Reviews, validates, monitors patient profiles to assure appropriate dosage and therapy as well as patient specific assessment of medication orders.

Reviews alerts associated with verification and escalates concerns to provider or pharmacy leadership as appropriate.

Supervises and performs compounding (both sterile and non-sterile), packaging, and dispensing of pharmaceuticals according to prescription, physician order, or floor stock request in compliance with state and federal law and regulation.

Handles all aspects of pharmacy operations to include order review and validation, sterile and non-sterile compounding, controlled substance/diversion management, pharmacy automation, and all state/federal law and regulation.

Establishes an environment of safety and assures safe practices for the patient as evidenced by recognizing and implementing safe medication practices, and following policies and procedures that comply with regulatory agency regulations.

Responsible for all core clinical pharmacy programs and commitment to evidence-based patient care, to including, but not limited to IV to PO, renal/hepatic dosing, therapeutic interchange, antimicrobial stewardship, anticoagulation management, duplicate therapy, drug-drug interactions, non-formulary management and weight based dosing.

Qualifications: Education Doctoral Degree, Pharmacy (Required) or Bachelors Degree, Pharmacy (Required)

Experience 1 year Prior pharmacy experience in area of practice related to the position (Preferred) 1 year PGY1 Residency (Preferred)

Requirements

Candidates must hold a Doctoral or Bachelor's degree in Pharmacy and possess a valid Virginia Pharmacist license. At least one year of relevant pharmacy experience or a PGY1 residency is preferred.
