We provide in home and community services for individuals with disabilities and other related conditions. Create tailored goals to meet the needs of individual clients. Turn in timely documentation including weekly invoicing, progress notes, quarterly reports, and related paperwork. Conduct Initial Evaluations (as needed for clients new to therapy), writing goals and objectives, and determining a recommended frequency of services. Ideal Candidate: Experience working with people with disabilities and/or dual diagnosis Vast knowledge of Massage Therapy techniques for varying impairments and/or disabilities Ability to create goals/ongoing treatment plans for clients Ability to travel to clients and provide in-home Massage Therapy services Essential
Must be able to travel to current or prospective clients’ homes Must be able to communicate with health care professionals, clients, families, and the general public through verbal and written communication Must have access to a private personal laptop or desktop computer for HIPAA compliance Track SMART goals and progress through weekly documentation using Microsoft Word, Excel, and Adobe Reader Implement patient care plans through goal-related activities Able to work independently, manage one’s own time and the time of client(s) All therapists must have the following: Resume LMT Certification CPR/AED Certification (American Red Cross or American Heart Association only) License to practice within the state of Texas Driver's License Social Security Card Professional Liability Insurance *Note: All contractors must submit to background and reference checks. *Visit our web site at www.chawtx.com to learn more about our programs
