About the role

The Care Coordinator supports patients and families by managing healthcare services, facilitating transitions of care, and ensuring adherence to treatment plans. They also track performance metrics and collaborate with clinical teams, payers, and community resources to improve patient outcomes.

The Care Coordinator is responsible for supporting patients and families by coordinating healthcare services, ensuring smooth transitions of care, and promoting adherence to treatment plans.

Working across multiple practice sites, this role focuses on care management and utilization/quality improvement, medication refill management, coordination of care for special and post-acute populations, and tracking of key performance and accreditation metrics, in close collaboration with providers, payers, and community resources.

ESSENTIAL FUNCTIONS: Care Management · Monitor patient progress and address barriers to care. · Facilitate communication between patients, clinical teams, and external partners. · Support utilization management and quality improvement activities.

Documentation & Compliance · Maintain accurate records in care management systems and EHR. · Ensure compliance with organizational policies and regulatory requirements. · Manage medication refill requests and processing in accordance with clinical protocols.

Patient Support & Navigation · Assist patient’s parents in understanding their care plans and available resources. · Coordinate appointments, referrals, and follow-up care across multiple providers. · Provide focused care coordination for special populations and post-acute care needs.

Resource Coordination · Connect patients with community services, financial assistance, and support programs. · Educate patients on preventive care and wellness initiatives. · Track and report on metrics related to accreditation, benchmarking, and progress toward program goals.

KNOWLEDGE, SKILLS & ABILITIES · Strong communication and interpersonal skills. · Ability to multitask and manage time effectively. · Proficiency in care management software and electronic health records (EHR). · Additional computer experience, including proficiency in Excel and databases. · Core Competencies: Empathy and patient advocacy, organizational skills, collaboration and teamwork, and problem solving.

EDUCATION & EXPERIENCE Bachelor’s degree in nursing, Licensed Practical Nurse certified, or a related field is preferred Minimum of 1-2 years of experience in care coordination or case management and or patient services preferred.

WORKING CONDITIONS General office working conditions.

May require occasional overtime during peak periods, such as month-end or year-end closings.

Manual dexterity is required to use desktop computers and peripherals.

Exposure to variable weather conditions is likely.

Requirements

Candidates should have a bachelor's degree in nursing or a related field, with a Licensed Practical Nurse certification preferred. A minimum of 1-2 years of experience in care coordination, case management, or patient services is also preferred.
