About the role

The Registered Nurse will provide clinical services, medication management, and case management for clients with severe mental illness and chemical dependency within the ACT program. They will also collaborate with the treatment team to conduct assessments, develop treatment plans, and provide on-call crisis intervention.

Working collaboratively with individuals, businesses, government and other service providers, St. Patrick Center will end chronic homelessness in the St. Louis region through Housing First, an evidence-based practice that tells us to remove barriers, do intake and assessment early, move people into housing and offer them support to achieve their goals.

Job summary

Patrick Center, is seeking a Registered Nurse.

This RN will provide professional, quality program services to the clients served by the Assertive Community Treatment (ACT) program.

In this role you will promote the physical and mental health of clients on the ACT team and work with other members of the treatment team to assist clients to obtain and maintain an optimum quality of life.

This position will provide clinical oversight to staff by monitoring, educating, coaching, and instructing clients as needed and will assist by providing treatment, rehabilitation, advocacy, and case management services to clients with severe and persistent mental illness and chemical dependency problems.

The core working hours are Monday-Friday, 8:00 AM-4:30 PM.

Overview and responsibilities

Provide consultation for ACT team regarding psychiatric symptoms, co-occurring medical conditions and appropriate psychiatric interventions.

Conduct comprehensive, ongoing assessment of program clients’ physical health and medical history (Nursing Assessment).

Manage primary case load by completing treatment plans, DLA-20’s and crisis plans in order to help with graduating clients to less intense level of care.

Provide direct clinical services to clients on an individual basis in the office and in community settings, making appropriate changes in treatment plan as indicated.

Refer clients for appropriate specialists or primary care physicians as indicated to promote recovery and overall health.

Collaborate with ACT team coworkers in daily staff meetings and treatment planning.

Take lead role and participate in providing medication administration and medical services.

Develop, maintain and supervise team psychopharmacologic and medical treatment and medication policies and procedures including administering, evaluating and recording psychotropic medications prescribed by the team psychiatrist; evaluate and chart psychotropic medication effectiveness, complications and side effects and arrange for required lab work according to protocol.

Administer Waived Testing - drug urinalysis testing.

Manage the system of getting medication to clients and integrating medication administration tightly into client ITP.

Administer IM medications to clients and complete required lab work as ordered by ACT doctor and/or Healthcare Homes.

Manage pharmaceuticals and medical supplies.

Collaborate/coordinate with other health providers for continuity of care, especially Affinia Health Clinic.

Work with ACT psychiatrist to coordinate care with ACT team and all existing social service agencies to encourage consistent and ongoing care of client.

Provide on call crisis intervention in coordination with other ACT team members, covering nighttime, weekend, and holiday hours.

Maintain service line client tracking system and client files including treatment plans, documenting client progress to maintain a permanent record within the timeframe as outlined by DMH and BJC/administrative agent.

Document notes in MyAvatar within 24 hours of intervention.

Ensure the care model by interacting with all other service line programs.

Responsible for ministry policy regarding work hours, building security and all written personnel standards.

Knowledge and experience requirements

Licensed Registered Nurse required.

Drug test within 30 days of hire required.

Requires driver license (MO residents require Class E), personal vehicle, and auto insurance.

Skills and competencies required

Understanding of mental health and substance use disorders.

Experience working with the unhoused community.

Knowledge of federal, state and agency policies of client confidentiality

Excellent communication skills, both verbal and written.

Values diversity and teamwork.

Demonstrates ability to learn and adapt.

Respect for clients and coworkers and ability to work collaboratively with the program team and all service line coworkers.
