The Case Manager provides home- and community-based services to homeless young adults to assist with housing stabilization and self-sufficiency. Responsibilities include conducting intake assessments, developing individual service plans, and coordinating with community resources and landlords.
The Case Manager 2 is responsible for providing home- and community-based case management services to homeless young adults located throughout the state. Responsibilities include; delivering comprehensive case management services to clients in their homes and in their communities, conducting intake assessments, assisting youth in locating available apartments for rent, working in partnership with youth to develop and implement individual service plans that will progress them to self-sufficiency, liaison with treatment providers and community resources, providing transportation to clients as needed, developing money management skills with caseload, developing employment readiness skills with caseload, liaison with landlords and property management staff to support youth’s continued tenancy, data entry and documentation, caseload size will vary between 8-12 young adults at any given time Working within our Young Adult Services, the Case Manager 2 works with the START program staff to provide services to stabilize and reunite families experiencing complex issues such as substance use, behavioral health matters, and homelessness.
Associate’s Degree 2+ years relevant experience in direct service and case management field Valid Connecticut Driver’s License Regular and predictable attendance is required Orientation: If selected for employment with The Connection, newly hired employees will be required to attend a two (2) day mandatory New Hire Orientation at our Roscommon office located in Middletown.
