Qualifications

Current California Physical Therapist (PT) License in good standing Graduate of an accredited Physical Therapy program Bachelor's Degree required; Master's or Doctorate in Physical Therapy preferred Experience in Skilled Nursing Facility (SNF), Post-Acute Care, Rehabilitation, Long-Term Care, or Acute Care settings preferred Strong patient assessment and treatment planning skills Excellent communication and documentation abilities Knowledge of EMR systems such as Casamba or PointClickCare preferred Ability to work effectively in a collaborative healthcare environment New graduates with strong clinical experience are encouraged to apply About Lakeport Post Acute At Lakeport Post Acute, we believe exceptional patient outcomes begin with exceptional clinicians. Requirements
