Responsible for screening clients for admission into our services, ensuring they meet the necessary criteria for substance abuse and mental health treatment.
Duties include conducting initial assessments, reviewing treatment consents, and explaining patient rights and responsibilities.
Manages the intake process, including financial reviews and documentation.
Using telehealth services for clinical screenings and counseling.
Performs various administrative tasks as assigned.
Care Coordination for clients to include intake, discharge planning, outreach, and linkages to other services both within and without PAR.
Timely documentation of all client information and screenings are documented in the agency's electronic health record (EHR) system.
Adhere to state, federal, and agency regulations regarding client confidentiality and service provision.
Maintain accurate records for auditing and quality assurance purposes.
• Current CPR and First Aid Certification or certifications must be obtained within the first six (6) months of employment.
Adheres to company time and attendance standards, with understanding of the needs of a 24-hour residential program if applicable.
Demonstrates knowledge of company policy and procedure.
Performs other duties as assigned and maintains a safe work environment.
