Pay range for this position is $23.42 - $34.10/hour.
Actual pay is based on years of licensure.
This is an Hourly position.
The Licensed Practical Nurse (LPN)- LPN Hospice collaborates with RN Case Manager to develop and implement the hospice plan of care. The LPN communicates with the patient’s provider and with the hospice inter-disciplinary team to meet the needs of the patient and family. The LPN is responsible for a scope of care that encompasses all ages from infancy through geriatric and demonstrates the knowledge and skills necessary to provide patient care that is appropriate to the ages of the patients served. The LPN provides evidence-based, patient-centered care at the patient’s place of residence, which may be in a private home or facility, using critical thinking and decision-making skills. The LPN has a thorough understanding of hospice documentation and their impact on outcomes of care with a focus on symptom management and comfort for both the patient and their family unit at the end of life.
Experience: Experience as LPN in a healthcare environment, Hospice preferred. Electronic Medical Record and Microsoft application basic proficiency. OSF HealthCare is an Equal Opportunity Employer.
Candidates must hold a valid LPN license, a BLS certification, and a valid driver's license with reliable transportation. Proficiency in computer skills and experience in a healthcare or hospice environment is preferred.
