Join Our In-House Therapy Team San Bruno Skilled Nursing is seeking a compassionate and motivated Physical Therapist (PT) to join our full-time, in-house rehabilitation team.
We are committed to providing high-quality, resident-centered therapy that helps individuals improve mobility, regain independence, and achieve meaningful functional goals.
Whether you're an experienced clinician or a recent graduate, you'll have the opportunity to work in a collaborative environment with support, mentorship, and a strong focus on resident outcomes.
Position Responsibilities Evaluate residents and develop individualized physical therapy treatment plans.
Provide skilled physical therapy services to improve mobility, strength, balance, coordination, and functional independence.
Establish measurable treatment goals based on each resident’s needs and abilities.
Monitor resident progress and adjust treatment plans as appropriate.
Document evaluations, treatment sessions, progress, and discharge recommendations accurately and timely.
Collaborate with nursing, occupational therapy, speech therapy, physicians, and other members of the interdisciplinary team.
Educate residents, families, and caregivers regarding therapy programs, safety, mobility, and home exercise programs.
Supervise and provide clinical guidance to Physical Therapist Assistants (PTAs), as applicable.
Promote resident safety, dignity, independence, and quality of life.
