Review patient profiles to assure safe and effective medication therapy Responsible for dispensing of medications, monitoring and reporting drug safety based on current clinical and institutional guidelines, as well as patient-specific criteria Oversees technician practice/activities as per regulatory requirements.
Responsible for mediation storage and documentation Actively participates in clinical services (e.g., stewardships, cost-effective, patient experience) Adheres to safety requirements in the handling and preparation of sterile and non-sterile medications and other hazardous agents (e.g., oncology) according to current guidelines (USP 797, USP 800, ASHP, NIOSH) Communicates effectively and appropriately with healthcare providers and caregivers and assures continuity of pharmaceutical care between shifts and among team members Participates in the training, education and mentoring of other pharmacists and technicians Participates in quality, safety and other project work related to medication management
Education/Experience Graduation from an accredited School of Pharmacy with PharmD required Current active registration and licensure with the State of New Jersey Board of Pharmacy required Previous hospital pharmacy experience preferred.
Candidates must hold a PharmD degree from an accredited school of pharmacy and possess an active New Jersey Board of Pharmacy license. Previous hospital pharmacy experience is preferred.
