Case Manager for Marion County
The Case Manager will provide education on the Medicaid waiver process and create individualized support plans based on the needs and goals of the individuals served. They will also facilitate community-based team meetings and help individuals access necessary services and supports.

Let’s talk about IPMG’s Case Manager position! This full-time, hybrid position, requires travel to community-based meetings multiple times per week, and requires working remotely from a home office setting.
This position requires in-person meetings; therefore, applicants must live within the State of Indiana or within 10 miles of the state line.
• Case managers work with Individuals on four Medicaid Waivers; Family Supports Waiver (FSW), Community Integrated Habilitation Waiver (CIH), Health & Wellness Waiver (H&W), and Traumatic Brain Injury Waiver (TBI)
Applicants must hold a Bachelor's degree in a human services field or have equivalent experience working with individuals with disabilities or the aging population. A valid driver's license, reliable transportation, and a designated home office setup are also required.
