Join our dedicated team as a Case Planner The Case Planner will work directly with children and families facing complex challenges, including serious medical conditions, substance use disorders, mental health concerns, and other circumstances that may impact family stability.
Through assessment, case management, advocacy, and resource coordination, the Case Planner will support families in achieving their goals and maintaining safe, nurturing home environments.
Conduct comprehensive assessments and develop individualized service plans. Coordinate multidisciplinary interventions tailored to each family's unique needs. Makes diligent efforts to meet the ACS minimum casework contact standards for a caseload of up to 13 families. Build meaningful relationships with families over an average case duration of 12 months. Collaborate with internal and external providers to ensure safety, stability, and well-being.
BSW or bachelor's degree in human services. Some knowledge of the child welfare system.
