The Case Manager will assess, plan, implement, and evaluate the needs of patients to ensure they receive appropriate services and support. This role involves collaboration with various stakeholders to facilitate effective discharge planning and improve client outcomes.
Hours: 8am-4:30pm
Requirement: Active RN in the State of IL, ADN
Advocate for patients’ needs and rights within the healthcare and social services systems., Conduct comprehensive assessments to determine patient needs, healthcare goals and eligibility for services., Coordinate services and resources, including referrals to healthcare providers, community resources, and support services., Delivers termination of benefits letters to patients as determined by third party payers, assuring patient understanding and awareness of rights and obligations. Assures understanding by family/significant others when patient is unable to understand/comprehend., Develop individualized care plans in collaboration with patients and relevant professionals., Maintain accurate and up-to-date records of patient interactions, assessments, and service plans., Provide patients and families with information about services, resources, and strategies to manage their conditions., Regularly monitor patient progress and adjust discharge plans as necessary., Work with multidisciplinary teams to ensure comprehensive care and support
